Case Management Associates LLC Health and Wellness Center is a unique Nurse Owned specialty hybrid clinic providing comprehensive Diabetes and Primary Care services as well as a wide range of educational resources regarding Diabetes and Heart Disease. Our providers have compbined more than 50 years of Nursing and Disease Management experience.
**STARTING IN SEPTEMBER 2022, WE WILL BE OFFERING DIABETES SELF MANAGEMENT/HEART HEALTH EDUCATION CLASSES ONLINE. (fees will apply)**
We accept most insurance, including Medicare and Medicaid.
We do not accept Humana, Cigna, Optima health or Virginia Premier care plans at this time
